Steps for Application to
MU's Department of Occupational Therapy
and Occupational Science

1. Review all prerequisites and admission requirements before applying.

2. Print forms and type in information . Read instructions carefully and complete all required forms. Forms are available below in pdf format.

   Part 2: Instructions and Checklist (2 pages)
   Part 2: Application Form (two pages, fill & print form)
   Part 3: Personal Statement
   Part 4: Activities
   Part 5: References (2 pages)
   Part 6: Observations (2 pages)
   Part 7: Transcript Request and Evaluation
