Checklist for the MHS Application

Application Materials that are sent to the Graduate School at the University of Missouri-Columbia:

1. ______ The Graduate School Application plus the application fee (applicants may submit a paper or on-line version)

2. ______ Official transcripts from every college/university attended (sent directly from the school's registrar's office)

3. ______ Official GRE scores (sent directly from ETS)

4. ______ Official TOEFL and TSE scores for international applicants (sent directly from ETS)

Application Materials that are sent to the Department of Communication Science and Disorders at the University of Missouri-Columbia:

1. ______ Three personal recommendation letters

2. ______ The CSD Department's Application for Admission

3. ______ The CSD Department's Financial Aid Application

4. ______ The Statement of Interest
